Cutaneous horn at the puncture site of arteriovenous fistula in a haemodialysis patient

چکیده
A 53-year-old woman with end-stage kidney failure from IgA nephropathy has been treated with haemodialysis for 14 years via the left forearm arteriovenous fistula (AVF). In August 2007, she developed an area of skin necrosis at the puncture site of AVF that was soon covered with a callus-like wrap. The puncture site was removed distally. In January 2008, a horn-like structure started to grow from the affected skin (Figure 1). Dimensions of the structure were 2.5 × 1.0 × 0.5 cm. The horn was removed in May 2008, during the surgical extirpation of AVF. Pathohistological finding demonstrated necrotic skin areas embedded by blood with partially preserved epidermis and signs of hyperkeratosis and parakeratosis. Arteriovenous fistula was constructed on the right forearm, and the patient continued with regular dialysis. To our knowledge, this is the first case of cutaneous horn in a haemodialysis patient. The horn resulted from hyperkeratosis, a physiological response of the skin to chronic extensive mechanical stress (repetitive AVF punctures). Although considered as benign lesions, cutaneous horns may evolve from neoplastic skin changes. A histopathological study of cutaneous horns demonstrated that almost 40% of these lesions may show (pre)malignant base pathology [1]. Fig. 1. Effect of Cryotherapyon Pain Following Fistula Puncture in Patients Undergoing Hemodialysis
Background and Objectives: Patients undergoing hemodialysis are exposed to stress and pain due to approximately 300 punctures per year they receive for their arteriovenous fistula. Alleviation of the pain can improve acceptance of the procedure and quality of life among the patients. This study aimed to assess the effect of cryotherapy on arteriovenous fistula puncture pain among patients uderg...
متن کاملSpinal dural arteriovenous fistula : a case report
Background: Spinal dural AVF is the most common type of spinal vascular malformation. However, presenting symptoms differ according to site of spinal involvement. This study described a case of arteriovenous malformation with paraparesis and incontinence. Case Presentation: Diagnosis of patient was confirmed by clinical and imaging examination using magnetic resonance image and ruling out ot...

Background: For patients requiring chronic hemodialysis, the preferred site for vascular access is an autogenous arteriovenous fistula. Although a properly formed fistula is advantageous because it is less susceptible than other types of vascular accesses to infection and clot formation and can last longer than any other types of vascular access, AV fistula has a high rate of early failure that...
متن کاملIatrogenic arteriovenous fistula in the groin presenting as cardiac failure
A patient presented with cardiac failure and investigations revealed a right femoral arteriovenous fistula (AVF) 5 weeks after the insertion of a femoral dialysis catheter for haemodialysis. No ultrasound had been used for catheter insertion. The iatrogenic AVF was repaired, and her episodes of cardiac failure ceased. It is recommended that femoral dialysis catheters are inserted using ultrasou...
متن کاملObesity is not an obstacle for successful autogenous arteriovenous fistula creation in haemodialysis.
BACKGROUND Obesity, which is often associated with diabetes, is increasingly encountered in the haemodialysed population, and this may produce difficulty in autogenous arteriovenous fistula creation. Prosthetic angioaccess or catheters, when used in place of autogenous fistulas, increase thrombotic and infectious complications in these already challenged patients. METHODS This prospective stu...
